About us

อภัย · Aphai

In Thai, Aphai means forgiveness. We believe everyone deserves a second chance at health — to step back, start again, and be cared for without judgment.

Dr. Andy Nakmanee

DO · Board-Certified Internal Medicine

Dr. Andy founded Aphai Primary Care to bring back something simple: a clinic where patients are people, not appointment slots. The practice pairs traditional, insurance-based adult primary care with a focused, cash-pay obesity medicine clinic — two services built on a single philosophy of unhurried, judgment-free care.

The name Aphai comes from the Thai word for forgiveness. It reflects the practice's core belief: that every patient deserves to be met without judgment and given a true second chance to take charge of their health — whether that means rebuilding after a setback, breaking a long pattern, or simply starting fresh.

Common Questions

Frequently Asked Questions

How does Aphai work?

Aphai is two practices under one roof. Adult primary care — physicals, sick visits, chronic condition management — is billed through your insurance, just like any traditional clinic. Obesity medicine runs as a separate, cash-pay clinic with simple flat fees: $75 for the initial consultation and $50 for every follow-up visit.

What insurance do you accept?

Most major insurance plans. Plans change often, so please call us before your first visit to confirm we're in-network for yours — or send us a quick message below and we'll check for you.

How much will a primary care visit cost?

Your cost depends on your insurance plan — copays, deductibles, and coinsurance work the same way they do at any in-network clinic. We'll bill your insurer directly, and you'll only owe what your plan determines.

Why is obesity medicine cash-pay instead of through insurance?

Most insurance plans either don't cover obesity medicine well, or they cover it with so many restrictions that real, evidence-based care becomes hard to deliver. By running obesity medicine as a separate cash clinic, we can offer comprehensive metabolic care — including GLP-1 medications when appropriate — without the gatekeeping. $75 for the initial visit, $50 for every follow-up, no insurance billing.

Are weight-loss medications included in the obesity medicine visit fee?

Your visit fee covers the medical care — evaluation, prescribing, monitoring, lifestyle counseling, and follow-up. The medications themselves (including GLP-1s like semaglutide and tirzepatide) are filled and paid for separately at the pharmacy of your choice. We'll help you find the most affordable path, whether that's through your insurance, manufacturer savings programs, or compounded options when appropriate.

Do you take Medicare or Medicaid?

Coverage varies by specific plan. Please call to confirm whether we're in-network for your Medicare or Medicaid plan before your first visit.

How do I schedule a visit?

Call our office, send us an email, or use the contact form below. New patients will receive a brief intake form to complete before the first visit.

How do medications work?

Prescriptions are sent electronically to the pharmacy of your choice. For primary care, your insurance's pharmacy benefit usually applies. For obesity medicine, prescriptions are paid out-of-pocket at the pharmacy — we'll always discuss cost openly and help you find the most affordable option.

Are vaccines and immunizations available?

For routine vaccines we don't stock in-house, we'll review what you need and point you to the most affordable option — usually a local pharmacy (Walgreens, CVS) or the county health department, where most vaccines are inexpensive or fully covered by insurance.

Do you offer in-office procedures?

Aphai focuses on internal medicine and obesity medicine — comprehensive, relationship-based primary care for adults. We don't perform in-office procedures such as biopsies, joint injections, or suturing. When you need one, we'll refer you to a trusted specialist and stay involved in coordinating your care.
